    Risk-management and rule-compliance are inter-related strategies for promoting safety in hazardous industries. They are co-existing and complementary, not contradictory. However risk-management offers very little guidance to end point decision-makers; they need rules to guide their decisions. Accordingly, it is important, even within a riskmanagement framework that risk-management be translated into rule-compliance for end point decision-makers, where possible. The paper demonstrates that this is what in fact happens for a wide range of operational decision-making.

    For non-operational decisions, such as investment and design decisions, the need to convert risk-management into rule-compliance is equally important, although more controversial. Nevertheless the authorities have shown that they are willing to impose prescriptive technical rules on duty holders in relation to non-operational decisions, in the interests of safety.

    These points are illustrated using a variety of empirical examples and materials, most particularly, the BP Texas City accident, the Buncefield accident, and the Australian pipeline standard.

    Upstream petroleum operations have the potential for rare but catastrophic incidents, such as blowouts, fires and explosions. The majority of such incidents are associated with a failure to effectively implement well known barriers or risk controls. The usual approach to managing barriers is to draft procedures describing what has to be done. This can lead to voluminous procedural documentation both difficult to understand and manage in its entirety. This is exacerbated by the varying quality of communications – invariably written from the perspective of the technical expert and not the person tasked with implementation. Asked to assist a Tier 2 oil production company, Peter was challenged to devise a different way to manage the barriers to major accidents.

    On March 23, 2005, the BP Texas City refinery experienced one of the most serious U.S. workplace disasters of the past two decades, resulting in 15 deaths, more than 170 injuries, and significant economic losses.

  • Lessons from Esso's Gas Plant Explosion - Longford
  •  

    In September 1998 Esso Australia’s gas plant at Longford in Victoria suffered a major fire. Two men were killed and the state’s gas supply was severed for two weeks, causing chaos in Victorian industry and considerable hardship in homes which were dependent on gas.

    What happened was that a warm liquid system (known as the “lean oil” system) failed, allowing a metal heat exchanger to become intensely cold and therefore brittle. When operators tried to reintroduce warm lean oil, the vessel fractured and released a large quantity of gas which found an ignition source and exploded.

    Professor Andrew Hopkins traces the reasons for this event, relying on evidence provided to the Royal Commission which investigated the disaster.
